Prescriptions
Please allow 24 to 48 hours for prescriptions to be processed. To request a prescription refill, please call the office during business hours. Please have the following information available when requesting a medication refill:
- Patient's name and date of birth
- Name of the prescribing physician
- Name of the medication
- Strength of the medication (i.e. 5mg)
- How many times a day you take the medication
- Name, location (city, state) & phone number of the pharmacy
- Phone number where you can be reached in case of any questions
Insurance & Methods of Payment
We accept most major insurance policies. You are responsible for co-payments, services that are not covered and deductibles. Payment is expected at the time of your visit. We accept cash, checks, VISA and Mastercard. Please understand that, while our staff is knowledgeable about what most insurance policies generally cover, we cannot be responsible for making sure that your insurance will cover the services you need.
We encourage all of our patients to understand which services their insurance company will cover and those for which they may be financially responsible. Coverage eligibility can be easily verified by calling the information numbers on the back of your insurance card. If you do not have insurance coverage, please call our business office at (978) 745-4767. We will be happy to assist you by arranging a payment plan.
Medical Records
All medical records are strictly confidential. We will release your records only with written authorization. No medical information will be given to friends, spouses or relatives without the written consent of the patient, parent or legal guardian. Please allow 5-7 business days for completion of your request.
School Forms
Upon request, we will complete school forms and worker's compensation forms for asthma, food and stinging insect allergies. Please allow 2 business days for completion.
